ABOUT THE CHA:
Every 3 years, the Hendricks County Health Department and Hendricks County Health Partnership work with our community partners to conduct a community health needs assessment. The Community Health Needs Assessment (CHA) is conducted to identify the diverse needs of the community that affect health and quality of life. The finding of the CHA are shared with our community members and service providers to bring awareness to the current needs of Hendricks County. This data also serves as a guiding tool that helps our service providers know which programs are adequately meeting needs, and what type of programs are still needed in our community.
WHAT IS THE CHA?
The CHA is a questionnaire type survey that anonymously collects data on different areas affecting health. This year’s community health assessment is estimated to take around 11 minutes to finish. At the end of the data collection period (August 1), the data will be analyzed and compiled into a report for the community’s reference. The survey is available in both English and Spanish.

What is the goal of the CHA?
The primary goal of the Community Health Assessment is to obtain a diverse and accurate health profile of our county that will be used to identify community needs. This will help our community know where to direct programming and resources.
Who can take the CHA?
Anyone who is at least 18 years old and lives or works in Hendricks County may participate in the CHA.
How long will the CHA be active?
The CHA will be available to take until August 31, 2022.
Is the CHA confidential?
All individual information collected in the CHA will not be shared or linked back to the survey-taker in any way.
What are the logistics of the Sweepstakes?
Those who so choose may enter our sweepstakes to win one of 5 $20 VISA gift cards, or one $15 Starbucks card. An email or phone number will be necessary to enter the sweepstakes, but will not be used for any other purpose other than contacting the winners to collect the prize won. Winners will be randomly selected and will have a week to claim their prize. If the prize is not claimed within a week, it will be redirected to another entry.
