Description Flourish Care Management is a comprehensive service designed to meet the long-term care needs of older adults and people of any age with disabilities in a home or community-based setting with better health outcomes, better care, and at a lower cost than institutional care. Using a team-based, person-centered approach, our care managers assess an individual’s eligibility for services, functional impairment and care needs, develop a customized plan of care, and coordinate all services required to extend independent living. |

Aged and Disabled Medicaid Waiver
Description
The Aged and Disabled waiver provides an alternative to nursing facility admission for adults and persons of all ages with a disability. The waiver is designed to provide services for people who would otherwise require care in a nursing facility. Waiver services can be used to help people remain in their own homes, as well as help people living in nursing facilities return to community settings such as their own homes, apartments, assisted living or adult family care settings. Description
The Community and Home Options to Institutional Care for the Elderly and Disabled program is administered through Indiana’s 15 Area Agencies on Aging (serving 16 planning and service areas). The CHOICE program provides home and community-based services to assist individuals in maintaining their independence in their own homes or communities for as long as is safely possible.

Brownsburg Older Adult Alliance
Description
BOAA’s programs and services have grown from an annual Holiday Drive providing hygiene and warm items for winter to holding weekly activities. BOAA arranges more than 10 monthly activities for seniors and seasonal service projects including Fall Leaf Cleanup and Holiday Gift-away. BOAA is not only a place for our seniors to connect with the community; in addition, we connect seniors in need with resources. BOAA’s goal is to assist seniors in creating their best life in Brownsburg. |
Chords of Love - Support Chatter

Chords of Love Support Chatter is a group started to help others like us with Cancer, Chronic and Terminal Illness and Seniors with Low Income in our community. We have a Facebook page for all that want to join this is a support Group! We are a 501c3 as of June 28, 2018! We are seeking donations to help others with food, medicine cost, and basic needs such as rent, food, clothing, medical supplies. 100% of Donations go to these patients in dire need of our help. We operate exclusively by all of the caring volunteers and donors. |

We provide care, nurturing and support for end-of-life guests and their families, bringing peace and assistance throughout a turbulent period. Our services come free of charge. We also host numerous community involvement activities, including craft workshops, classes, gardening and support and bereavement groups |

Description
Project Lifesaver is a proactive response to identify those individuals within the community who have a tendency to wander away from their caretakers. This program consists of a small radio transmitter that the client wears on their wrist or ankle. Each transmitter has a frequency that is unique to the client. They can then be tracked by receiving equipment that highly trained emergency responders use.

Provides door-to-door transportation for Hendricks County citizens to get to necessary services such as medical appointments, grocery shopping, legal services, social services, financial services, and so on. Vehicles can accommodate wheelchairs, portable oxygen, attendants, and service animals. Assistance boarding and unloading is provided. Appointments are required. The service is a partnership of Hendricks County Senior Services and Sycamore Services. |

Guardian Angel Hospice
Description
For residents of Hendricks County, Guardian Angel Hospice provides medications, equipment and supplies related to terminal illness. We offer our trademarked Angel Touch therapeutic touch with high grade essential oils and lotion for comfort at the end of life, as well as emotional, spiritual, and personal support for patients, caregivers, and other members of the family. Grief and bereavement support is available for family members to find comfort during and after their loved one’s final journey. Payment for hospice services is covered 100% by Medicare and frequently covered by private insurance. With generous donations from the community, we are also able to provide services to those who are not eligible for Medicare. No one will be denied care because of an inability to pay. |

Description
The Adult Protective Services program was established to investigate reports and provide intervention and protection to vulnerable adults who are victims of abuse, neglect, or exploitation. CICOA Aging & In-Home Solutions has established local, independent councils on aging in each of the eight counties it serves. Each council is comprised of dedicated volunteers who help define the problems and needs of older Hoosiers in their county and identify solutions. Membership is free and open to anyone or any organization that has a sincere interest in the welfare and well-being of older adults in Hendricks County.
