Short-Term Rehabilitation is designed for those striving to restore abilities due to stroke, cardiovascular difficulties, orthopedic surgery and other debilitating conditions. Our goal is to help participants return home safely following a hospitalization with the skills they need to continue life on their own terms.
Our person-centered program is tailored to the individual. Short-Term Rehabilitation includes physical, occupational and speech/language therapies and a full range of interdisciplinary support, including doctors, nurses, therapists, social workers and other service care providers. From therapy and medically integrated programs to our holistic wellness services, we tailor a care plan to improve strength, independence and quality of life.
Services include wound care, rehabilitation screenings, fall prevention, nursing, contracture management, wheelchair position, mobility training and walking ability programs. Add this to our highly successful strength-building wellness and aerobic conditioning program, and you have an incredible combination that will truly help you move forward!- Physical Therapy focuses primarily on independent mobility, regaining strength and balance, using proper body mechanics and energy saving techniques to better perform mobility tasks.
- Occupational Therapy promotes independence with daily living activities like bathing, eating, dressing, self-care and home management tasks. Participants gain strength and coordination while learning to conserve their energy.
- The goal of Speech/Language Therapy is to increase functional communication, cognitive skills and teach safe swallowing by introducing diet modifications and special feeding techniques. Therapy may include muscle retraining, compensatory strategies and use of communication devices.

Hospice Services
Our interdisciplinary Care Team is comprised of Physician(s), a Hospice Medical Director, Nurses, Social Workers, Home Health Aides, Chaplains, Counselors and Volunteers from your own community. Our team works closely with your current Physicians and to create a care plan that addresses your clinical and emotional needs. We recognize that hospice care does not abide by standard hours of operation. A Care Team representative is available to you and your loved ones, 24/7 including holidays, ensuring the best possible experience and quality of life for all.
Routine Care
Routine home care is the most common hospice service utilized. Hospice services are provided within the patient’s home, whether it’s a private residence, assisted living facility, or long-term care facility. All services are provided in the home and nurses, aides, counselors, etc. will visit you to ensure your comfort and safety.

Respite Care
Respite care is a short-term inpatient stay that provides the family and/or caregiver relief, for no more than 5 days in a month. Respite care must be given in a Medicare designated inpatient facility, and may not be provided in an assisted living facility or private residence. Respite care is available if your family or caregiver is physically/emotionally drained, ill, or has an event they must attend such as a wedding. There must be a designated caregiver in order for respite care to be considered.

When medically appropriate, and a more intensive level of care is required to help manage symptoms, our hospice team members will work in shifts of up to 24 hours per day for a brief period to help you remain at home, rather than be admitted to the hospital. This intensive level of care is appropriate when a patient is having acute symptoms that can’t be managed by the primary caregiver or with intermittent visits.

- Rehabilitation
Short-Term Rehabilitation is designed for those striving to restore abilities due to stroke, cardiovascular difficulties, orthopedic surgery and other debilitating conditions. Our goal is to help participants return home safely following a hospitalization with the skills they need to continue life on their own terms.
Our person-centered program is tailored to the individual. Short-Term Rehabilitation includes physical, occupational and speech/language therapies and a full range of interdisciplinary support, including doctors, nurses, therapists, social workers and other service care providers. From therapy and medically integrated programs to our holistic wellness services, we tailor a care plan to improve strength, independence and quality of life.
Services include wound care, rehabilitation screenings, fall prevention, nursing, contracture management, wheelchair position, mobility training and walking ability programs. Add this to our highly successful strength-building wellness and aerobic conditioning program, and you have an incredible combination that will truly help you move forward!- Physical Therapy focuses primarily on independent mobility, regaining strength and balance, using proper body mechanics and energy saving techniques to better perform mobility tasks.
- Occupational Therapy promotes independence with daily living activities like bathing, eating, dressing, self-care and home management tasks. Participants gain strength and coordination while learning to conserve their energy.
- The goal of Speech/Language Therapy is to increase functional communication, cognitive skills and teach safe swallowing by introducing diet modifications and special feeding techniques. Therapy may include muscle retraining, compensatory strategies and use of communication devices.
We monitor our senior rehabilitation outcomes and use the data to consistently improve our services.
- Memory Care
While a cure for Alzheimer’s disease does not yet exist, many options are available that enable those with the disease or other forms of memory loss to enjoy a higher quality of life. The progression of Alzheimer’s and the symptoms that accompany it often make it difficult for family members to continue care at home. In response, we’ve created a memory care program specializing in serving each individual through their unique experience with memory loss.
Our memory care is a structured, research-based memory care program. We utilize a person-centered, wellness-based model of care that incorporates stimulating daily activities to sustain independence and promote a healthy spirit.
